The flu vaccine is required for those at Loyola's Health Sciences Campus

Flu shots are mandatory for faculty, staff, and students at the Health Sciences Campus.

Proof of compliance must be uploaded into the Loyola Health App. Upload now

HSC students: There are a limited number of free flu vaccines available at the HSC Wellness Center

How do I access and use the Loyola Health App?

Loyola Health can be accessed via desktop or web browser here.  Or, access it on your phone via the Loyola Health function on the Loyola Mobile app. 

After logging in with your UVID and password, click on the Upload Vaccinations tab.

 

 

 

Select FLU VACCINE from the Vaccine Type drop down box.

Choose the type of Flu Vaccine that you received from the Vaccine Name drop down box.  (The first several options are the types of vaccines offered on Loyola campuses.) 

 

Enter in the date you received your flu shot and upload your documentation.

Click Submit.

I'm a HSC student. How can I get a free flu shot on campus?

HSC students can schedule their flu vaccine at any Wellness Center location.

I'm faculty or staff. How can I get a free flu shot on campus?

Faculty and Staff may sign-up to receive a free Flu vaccine at the Lake Shore, Water Tower, and Health Sciences campuses in October and November. To schedule an appointment, please visit Empower Health. Please note, the Empower Health portal no longer requires a site code as you will use your UVID ID and password to access your account and additional information about free Flu vaccines is available online. If you have questions about the LUC Flu Vaccine mandate for the Health Sciences Campus, please email EmployeeImmunizations@luc.edu.

What documentation do I need to upload as proof of my flu vaccine?

Valid proof of flu vaccine is documentation that includes your name, DOB, type of flu vaccine that you received, lot number, and date the vaccine was administered.  If you receive a flu shot from your primary care provider, this information can be found on your electronic health record.  Other flu vaccine providers (ie, a CVS Pharmacy) will also give you documentation with this information.

If you’re an employee and receive a flu shot through Empower Health, documentation will be submitted automatically.
