Appointments & Services

All nutrition appointments for the Wellness Center must be made by calling or online. The Wellness Center does not accept walk-in appointments. To schedule an appointment please call Dial-A-Nurse at 773.508.8883 or schedule online.

*Please arrive 15 minutes early to initial appointments in order to complete screening surveys upon check-in.*

Nutrition appointments can be conducted in person at the Lake Shore Campus or via video conferencing. If video conferencing is preferred the dietitian will provide you with a unique link prior to your appointment time.
